Self-Declaration form (SDF) Guide

To help us understand your needs and offer the right support, we invite you to complete the Self-Declaration Form (SDF) on your e:Vision account.

Follow these steps to complete the SDF:

  1. Log into e:Vision

  2. Click on the Accessibility, Disability & Inclusion tab in the main menu

  3. Accessibility, Disability & Inclusion page will load
  4. Select Self-Declaration Form (SDF)

  5. Complete the form and, if you have supporting evidence (e.g. medical documentation), upload it under the relevant header at the end of the form

 Don’t worry if you don’t have your evidence ready yet. You can still submit the form — we’ll be here to guide you as you gather anything needed later.

 

What Happens Next?

The information you share helps us create your personalised Tutor Awareness Sheet (TAS) — a confidential document that outlines any reasonable adjustments and support recommendations, such as extra time in exams or coursework extensions.

We treat your information with care and integrity. It is only processed and shared with your consent, and only with the staff who need to know in order to provide your support.
