This Cancer Care Management module is for registered health care professionals working in community /primary care settings.
The aim of this module is to equip Registered Health Care Professionals working in diverse community settings to understand the journey that individuals diagnosed with cancer undertake and the key role that community staff have in supporting these individuals and their families. Developed in collaboration with MacMillan Cancer Support and Wolverhampton CCG, this short course will focus on the three key elements of the patient’s journey:
a) The initial contact with services focusing on the journey from cancer free to an individual facing cancer treatment.
b) Supporting the patient and their families through the cancer treatment phase of their journey.
c) Helping the patient and their family to live well with and beyond cancer.
The course will be delivered on a fortnightly basis for a total of 6 study days and the module is offered at both level 6 (6HW137) and level 7 (7HW114).
